The 29-year-old, who joined Kingsmeadow from Swedish team Linkopings in 2017 and has since won four Women's Super League championships, broke the news to fans in an emotional statement posted on the club's Twitter account.
She has played 149 times for Emma Hayes' team and hopes to add another WSL championship to the FA Cup she won on Sunday to finish a trophy-filled run at the club.
With two games remaining—away at Arsenal and at home against Reading—Chelsea leads Manchester United, whom they defeated 1-0 in the Wembley championship match, by two points.
Eriksson will have won 12 trophies in total during his time at the club should they defeat United and win the championship.
Eriksson, who was crying, added, "It's really difficult even to talk about and say out loud." "It seems somewhat bizarre. I have some news for the world and the fans: I'll be leaving Chelsea at the end of the current campaign.
"This has been the best period of my life; it has been fantastic. It's been six incredible years with my teammates, the supporters, and the club as a whole.
"I feel so lucky to have traveled this path with the club, as we've really made a name for ourselves as one of the best clubs in the world," the member said.
Chelsea lost to Barcelona in the Champions League semifinal in April, ending its chances of winning an unprecedented treble.
But the skipper still has a chance to polish off her stay in west London by leading her team to a fourth domestic double in six years.
Even if it may seem like it, she continued, "I feel like I'm not only sad because it's been such an amazing journey."
"I'm also just so incredibly proud of the entire adventure and everything we've been through together. There are definitely mixed feelings.
